USMLE Step 1 prep courses, review classes and programs

USMLE does not run or endorse any USMLE Step 1 prep course: it states that third-party materials and courses are not affiliated with or sanctioned by the USMLE program. What USMLE offers is free: the content outline, more than 100 sample questions and a software tutorial. NBME adds paid self-assessments from $62. Any course you buy should be measured against those official materials.

Is there an official USMLE Step 1 course?

No. Neither USMLE nor NBME offers a USMLE Step 1 course or class for candidates. USMLE’s notice on review courses says that commercial materials and courses exist, that none are affiliated with or sanctioned by the USMLE program, and that information about them is not available from ECFMG, FSMB, NBME or the USMLE Secretariat.

The same notice is clear about one red line: it is unlawful for any test preparation program or individual to use, disclose or distribute actual USMLE questions or answers. A course that advertises real exam questions is a reason to walk away, not to sign up.

The USMLE Step 1 course outline you should compare against

The best USMLE Step 1 course outline is USMLE’s own content outline. Step 1 organizes basic science along two dimensions, system and process. Pathology (45–55%) and physiology (30–40%) are the largest disciplines, and applying foundational science concepts makes up 60–70% of the exam.

Before you pay for any USMLE review course, put its syllabus next to the official outline and check that the time it spends on each system and discipline is roughly in proportion.

USMLE prep courses and programs: the main formats

USMLE prep courses come in a few formats: live online classes, recorded lecture series, self-paced programs built around a question bank, and a USMLE Step 1 prep course in person. Which format suits you depends on how you learn and how much structure you need.

Whatever the format, the exam itself is fixed: up to 280 questions in fourteen 30-minute blocks over one 8-hour day. USMLE prep programs that train you on timed 30-minute blocks match that rhythm.

How to choose the best USMLE Step 1 prep course

There is no official ranking, so the best USMLE prep course is the one that fits your gaps. USMLE advises examinees to understand what a service offers, where its content comes from and what its claims are based on. Use these questions when you compare a USMLE preparation course:

  • Does the syllabus follow the official Step 1 content outline and its weights?
  • Who writes the questions, and does every answer come with an explanation?
  • Does it measure your progress by content area, not just give an overall percentage?
  • Does it practise the current format of 30-minute blocks of up to 20 questions?
  • Are its claims, such as pass rates, explained and verifiable?

Free and official alternatives to USMLE courses

Before you buy USMLE courses, use what is free: the content outline, more than 100 sample questions as a PDF and as an interactive test, and the testing software tutorial. Once you have your scheduling permit, you can also take a practice session at a Prometric test center for a fee.

For progress checks, NBME sells the Comprehensive Basic Science Self-Assessment: seven forms of four 50-question sections, from $62, with an estimated probability of passing Step 1 on the score report.

Certsqill’s USMLE Step 1 question bank is expert-developed, with an explanation for every answer option and a readiness score per content area, so you can combine it with any course or with self-study.
