NREMT exam: certification, cost, practice and prep
The NREMT exam is the national certification examination of the National Registry of Emergency Medical Technicians, offered at four levels: EMR, EMT, AEMT and Paramedic. For EMTs it is a computer adaptive cognitive exam of 70 to 120 items in 2 hours, taken at Pearson VUE or online, and the application fee is $104. A psychomotor exam, approved by your state, is separate.

What is NREMT certification?
NREMT certification is the national EMS credential issued by the National Registry of Emergency Medical Technicians after you pass its NREMT certification exam. There are four levels: Emergency Medical Responder (EMR), Emergency Medical Technician (EMT), Advanced EMT (AEMT) and Paramedic.
An NREMT certificate is not the same as a license to practice. Your State EMS Office issues the license, and the National Registry tells you to contact that office to verify a license. You need to recertify every two years to keep the national certification.
The NREMT certification search on the National Registry’s Verify Credentials page lets you look up a national certification by registry number, EMS ID or name. It confirms national certification only, not a state license.

The NREMT cognitive exam for EMR and EMT
The updated EMR and EMT certification examinations went live on 7 April 2025. Both are computer adaptive: the number and difficulty of items vary from candidate to candidate, while the passing standard stays the same. The EMT exam has 70 to 120 items, 10 of them unscored pilot items, and a 2-hour limit. The EMR exam has 90 to 110 items, 30 of them unscored, in 1 hour 45 minutes.
The EMT test plan sets five domains: Scene Size-up and Safety 15 to 19%, Primary Assessment 39 to 43%, Secondary Assessment 5 to 9%, Patient Treatment and Transport 20 to 24%, and Operations 10 to 14%. Pediatric care is integrated throughout the exam.

What NREMT questions be like: item types
If you have seen posts about what NREMT questions be like, here is what the National Registry actually lists. The EMT exam uses multiple choice, multiple response, options tables, build list and drag-and-drop items. Build list asks you to arrange options in order; drag-and-drop asks you to sort options into categories; option or check-box items ask you to classify options in a table.
On the paramedic exam, some items are linked to a scenario, and the exam also contains multiple-response and technology-enhanced items. Because the exam is adaptive, the difficulty of each next item depends on how you answered the previous ones.

The NREMT paramedic exam
The NREMT paramedic test is computer adaptive with 110 to 150 questions, 20 of which do not count toward your score, and a time limit of 3 hours 30 minutes. Scores are reported on a scale from 100 to 1500.
Besides standard multiple choice, the exam contains multiple-response and technology-enhanced items, and some items are linked to a scenario. You cannot tell which questions are unscored, so treat every one as if it counts.

Paramedic exam since 2024: NREMT static cardiology practice strips and ECG items
Since 1 July 2024 the NREMT paramedic exam is one examination with no psychomotor examination required. Its six domains are airway, respiration and ventilation; cardiology and resuscitation; medical, obstetrical and gynecology; trauma; operations; and clinical judgment, which makes up 34 to 38% of the exam.
If you are searching for NREMT static cardiology practice strips: on the current paramedic exam, 30% of the Cardiology and Resuscitation domain is allotted to graphical items with ECG rhythm strips. Rhythm recognition is therefore tested inside the cognitive exam, not at a separate station.

The NREMT psychomotor exam
For EMRs and EMTs, the NREMT psychomotor exam is a separate skills examination. The National Registry states that the BLS psychomotor process is established and approved by each state and did not change with the 2025 cognitive exam update. EMT handbook rules say state-approved BLS skills competency results are valid for 24 months.
AEMT and paramedic candidates no longer take a National Registry psychomotor exam: since July 2024 their certification is a single examination.

NREMT exam cost
The NREMT exam cost is an application fee charged per attempt: $88 for EMR, $104 for EMT, $159 for AEMT and $175 for Paramedic. Rescheduling or cancelling through Pearson costs $30, and a rescoring request costs $150.
The NREMT test cost does not include your EMS course or any state skills examination. Those are set by your program and your state.

Eligibility, NREMT test sites and scheduling
You need a completed state-approved initial course; initial courses are valid for two years from the month of completion. Once your application is complete, the National Registry issues an Authorization to Test (ATT), valid for 90 days.
NREMT test sites are Pearson VUE testing centers, or you can test remotely through Pearson’s OnVUE online proctoring. Changes must be made at least one business day (24 hours) before your appointment.

Retakes and NREMT refresher courses
If you do not pass the EMT or paramedic exam, you wait 15 days from your last exam before testing again, and you have six attempts in total. After three failed attempts you must complete remedial education before the fourth application. EMR candidates have three attempts; after that, a new full EMR course is required.
For EMTs, the remedial training requirements can be met with the 20-credit National Component of the NCCP model or a state- or CAPCE-approved NREMT refresher course. Some states require a traditional refresher, so check with your State EMS Office.

NREMT recertification requirements
The NREMT recertification requirements follow the National Continued Competency Program: half of the hours come from the national component, a quarter from local or state requirements and a quarter from individual education. Certification runs for two years and expires on 31 March.
Any NREMT recertification course you take should map to these components. EMTs can also recertify by examination instead of documenting continuing education.
- EMR: 16 hours (8 national, 4 local or state, 4 individual)
- EMT: 40 hours (20 national, 10 local or state, 10 individual)
- AEMT: 50 hours (25 national, 12.5 local or state, 12.5 individual)
- Paramedic: 60 hours (30 national, 15 local or state, 15 individual)

How to pass the NREMT
There is no minimum percentage of correct answers on an adaptive exam, so how to pass NREMT exams comes down to consistent performance at the passing standard across every domain. The National Registry does not recommend a particular study guide and says no single source fully prepares you. Your course textbook, the current AHA guidelines and the published test plan are the core.
Good NREMT test preparation covers the content and the format: technology-enhanced items, scenarios at paramedic level, and the 2-hour pace at EMT level. NREMT classes and an NREMT course from your program teach the content; an NREMT prep course or NREMT basic test prep adds exam practice. For NREMT paramedic prep, rhythm strips and clinical judgment deserve extra time.

NREMT practice test and practice questions
Certsqill’s NREMT practice questions are expert-developed and mapped to the National Registry domains, with an NREMT EMT practice test, an NREMT paramedic practice test and an NREMT cognitive exam practice test by domain. Use them as NREMT exam prep alongside your course, as NREMT study questions or NREMT sample questions by domain.
Every NREMT practice test question comes with an explanation for each answer option, and your results build a readiness score per domain, so you know which NREMT questions you still miss. A 7-day money-back guarantee applies.
